The Children's Medical Security Plan (CMSP) is a MassHealth program that is managed
by MassHealth and Unicare.
You will get a MassHealth Eligibility Review Form (ERV) in the mail. It is
important that you fill out and return the MassHealth form right away. MassHealth needs this
information to make sure you and your family are still eligible for CMSP benefits. You might
even be eligible for other MassHealth coverage, which may give you more health services.
If you do not fill out and return the form to MassHealth within 60 days of getting
it in the mail, you and your family will lose MassHealth/CMSP health insurance coverage.

What non-citizens need to know about applying for MassHealth, CMSP, Healthy Start, and the Uncompensated Care Pool
- MassHealth is a government program that pays for the costs of certain kinds of medical care.
You may be able to get MassHealth to pay for your health care. To see if you can, you must
submit an application form and show you meet certain financial and non-financial eligibility rules.
The MassHealth application form is also used to apply for other MassHealth programs that are available to
more people but pay for fewer kinds of medical care.
- To get the type of MassHealth that covers the most benefits, a non-citizen must submit documentation
about his or her immigration status. A non-citizen can also use the MassHealth application to apply for
other MassHealth programs that do not require any information about your immigration status, but pay for
fewer medical services.
- To apply for any kind of MassHealth coverage, you must be a resident of Massachusetts.
This means you actually live in Massachusetts and are not temporarily visiting here.
- If you submit copies of immigration documents or other proof showing that immigration officials
know you are living in the U.S., you may be able to get more health benefits. To find out which non-citizens
are eligible for full MassHealth, go to the MassHealth website at http://www.mass.gov/masshealth.
- A non-citizen does not have to submit any immigration documents with the MassHealth application form if:
- you are applying for your U.S. citizen children, but are not applying for any benefits
for yourself; or
- you are only applying for MassHealth Limited or certain MassHealth programs that have more
limited benefits but are available regardless of immigration status.
- To apply for MassHealth Limited or other MassHealth programs that do not require proof of your
immigration status (or if you are only applying for your U.S. citizen children):
- you do not need to give us any immigration documents;
- you do not need to have a social security number; but
- you do need to give us proof of your income. If you do not have pay stubs or tax records,
you can prove what your income is in other ways, like giving us a signed statement.
- Your application and the information on it will be kept confidential.
- Your name and address will not be sent to immigration officials.
- If you do not have a social security number, MassHealth will not match your information with other agencies.
- Getting health care under a MassHealth coverage type or program will not make it harder for
you to get a green card (unless you live in a nursing home). For more information about the

- If you are helping someone who does not speak English, MassHealth can provide interpreters to
help that person apply. Call a MassHealth Enrollment Center at 1-888-665-9993 (TTY: 1-888-665-9997
for people with partial or total hearing loss). There are also two member booklets that explain MassHealth,
which are available in eight languages.
